All athletes, grades 7-12, who participate in any Coweta school sponsored sport will be required to have an OSSAA Physical Form, a Concussion and Head Injury Acknowledgement Form, an Athlete/Parent/Guardian Sudden Cardiac Arrest Symptoms and Warning Signs Information Sheet and Acknowledgement of Receipt and Review Form, and a drug test as outlined by the Coweta Board of Education Policy.
